We tested here, and after applying the stylesheet for XHTML in the output it is inserted a link. In case of HTML or PDFs there is nothing inserted. I think this is a problem of the Docbook stylesheets, please take a look at the site: http://docbook.sourceforge.net/projects/xsl/index.html Maybe you can contact the authors for further information.
