Thanks for the sample, we'll fix this on our side. If you want a quick fix, open the CSS file "OXYGEN_INSTALL_DIR\frameworks\docbook\css\elements.css" and replace this CSS selector "book {" with ":root {"

Chapters are not proberly numbered, and the section ref to the chapter is zero.
