On a hunch, I opened up the same project and sample docbook 5 file in XML Author instead of XML editor. I was able to get the default epub transform to work. Alas, there are slight differences in the command to run the executables (i.e., the names of the files, and the order in which they are mentioned). Any idea why this would make a difference?
